Welcome to White Rose Surgery
We are pleased to accept registration requests from patients who are living within the area indicated on the map below.
Enter your postcode using the search option to see if you live in our catchment area.
If you live here, or are moving into this area, you are welcome to apply to register with our practice.
How to register with us
Alternatively, to register at the practice you will need to attend to fill in the relevant registration forms. We will require a form of identification with your current address on it e.g. utilities bill, and your photo ID eg. drivers license or passport. You will also need to book an appointment with the Health Care Assistant/Practice Nurse for a new patient medical, which will take about 10 minutes.
