Services

We provide a number of services to our patients within a primary care setting, including a Virtual Ward program

The virtual ward service is aimed at preventing emergency admissions amongst the over 65yrs population who may be housebound, vulnerable or suffering from one or more long term conditions.  The service also optimises the input and interventions for these patients, hopefully improving their quality of life and general health.  Patients are assessed by the team following either a direct referral, a recent hospital discharge, decline in health or by using our risk analysis tools.  Virtual Ward contact number - 01977 655601.
